docs: update PROJECT_TRACKER to reflect Phase 3 completion

Browse Source 
All 23 tasks across Phases 0-3 are now complete. Phase 3 items
(Analytics, AI/ML Services, MCP Servers, Monitoring) were already
committed but tracker was not updated.
